Payment & Insurance Information

 



Informing our patients about our policy and their financial responsibility assists us in providing the best services to our patients. Thank you for taking the time to read this page. Medicare 
We participate with most Medicare plans. Co-payments, deductibles and non-covered services are the responsibility of the patient and payment is expected at the time of service. We will bill your secondary insurance if you provide us with the insurance information at the time of your visit. You will be responsible for your coinsurance if reimbursement is not received within 30 days of submission to your secondary insurance.


Medicaid

We participate with most Medicaid plans and will file for you when it is your primary insurance. Your co-pay will be due at the time of your visit. We do not file Medicaid as a secondary insurance.

 

Insured Patients                      
We will bill your visit directly to your insurance provider if you supply an updated proof of insurance at each visit. Should your insurance require a referral or authorization, you will be responsible to provide the proper forms prior to your visit. Please note that the insurance policy is between you and your insurance provider. We are happy to file the claim for you, but the balance is your responsibility if your plan does not pay after 30 days. Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us.

 

Self Pay
A discounted fee will be due in full at the time of service. If there is a remaining balance, it will be promptly refunded.

 

Hospital Services        
If our physicians provided care to you in the hospital, your insurance will be billed regardless of our participation agreement. If we do not participate with your insurance, you are responsible for payment if reimbursement is not received within 30 days of submission. Patients are responsible for all co-payments, deductibles and co-insurance. Should you require care in our office after your hospitalization, you will be required to pay any remaining balance at the time of your appointment. It is necessary for you to check with your primary care physician and/or your insurance company for the rules of your particular plan before your appointment with us is scheduled. (i.e. managed care companies require a referral).

Travel Medicine
All services provided by our sister company, Travel Medicine, LLC, are payable at the time of service. Travel Medicine, LLC, is not a participating provider with any insurance company. Therefore, we cannot file insurance or accept assignment for these services.

 

Office Infusion Center
All services are billed through our infusion billing company. Please see the infusion section under Ancillary Services.
